•Carries a caseload as assigned by the Borough Coordinator or Assistant Borough Coordinator.
•Ensures that all participants receive quality program services and referrals to other community services within the policies and guidelines.
•Is directly responsible for accessing care coordination services for the participant including comprehensive care management, care coordination and health promotion, comprehensive transitional care, patient and family support, and referral to community and social support services
•Assures timely intakes, assessments and referrals.
•Coordinates all assessments from the various disciplines, developing case plans, and referrals for treatment.
•Maintains all case records in direct accordance with company standards of practice.
•Discusses the program and services offered to the participant, from the point of intake to discharge.
•Establishes and maintains a case record on participants.
•Obtains all necessary records from hospitals, doctors and other agencies.
•Responsible for meeting with the participant in the field on an as needed basis for follow up and counseling.
•Drive facility vehicle when the need arises (escort, field trips, etc.).
- A Masters degree and a minimum of two years experience or a Bachelors degree and four years experience in Human Services required.
- Valid and clean New York State driver's license and access to a vehicle required.
