Nurse - part time
•Performs or coordinates pre-admission evaluations and screenings of prospective consumers within prescribed guidelines. Assess consumers clinical problems and nursing care needs. Records physical findings; formulates plan and prognosis based on consumers condition and needs. If an LPN, assessments will be performed under the direction of the team RN or psychiatrist.
•Submits care plan and goals of individual consumers for periodic review and evaluation by Psychiatrist.
•Transcribes Psychiatrists medication orders; administers prescribed injections and medications daily. Observes consumer, records significant conditions and reactions, and notifies Psychiatrist of consumers condition and reaction to drugs, treatments, and significant incidents. Instructs consumers about daily medications (e.g., names of drugs, dosages, interactions, benefits). Reviews medication record of each consumer for potential adverse reactions, allergies, interactions, and contraindications on a monthly basis. Review Medications for errors.
•Provides on-going care and treatment of medical issues following physicians instructions and/or treatment plan.
•Records vital statistics and other pertinent data on consumers; maintains records of same.
•Provides first aid and daily nursing care to consumers, as needed. Takes temperature, blood pressure, pulse, weight, and other vital signs to detect deviations from normal and determine progress of consumers.
•Liaisons with physician, dietitians, hospitals, pharmacies, and the consumers family on all medical situations.
•Administers Hepatitis Vaccine and PPD test to staff, as scheduled.
•Reviews special dietary orders.
•Utilize the agency vehicle to conduct visits, escort consumers to appointments, transport food or goods for the consumers home or program sponsored activity.
•Conduct field based community visits. The ACT Program provides 85% of their services in the community.
•Instructs staff in methods and procedures, including, but not limited to: detecting signs and symptoms of illness; specific health needs of the consumer; first-aid; proper oral hygiene; nutrition infection control; universal precautions; OSHA standards; hospitalization and emergency medical procedures.
•Attends interdisciplinary team meetings; OMH approved ACT orientation and training; and Infection Control Committee meetings regarding the control of communicable disease and infections; collaborates in the design of, and implements approved, protective and preventive measures.
•Perform any other service provision tasks assigned by the Team Leader.
RN or LPN, and at least one year of experience working with the mentally ill.
Valid and clean driver's license.
