ORGANIZATION: Weston United Community Renewal, Inc. is a not for profit organization located in Harlem, New York. For over 30 years, Weston United has provided housing and supportive services to participants with histories of chronic homelessness, mental illness, and substance abuse. Weston United employs about 100 employees who contribute to a rich and diverse work environment.
SUMMARY: The Daily Living Skills Specialist will be responsible for providing case management services to a caseload of 15 to 20 residents and reports to the Program Coordinator.
ESSENTIAL DUTIES: Provide weekly individual counseling and case management services (face to face) to residents. Assist residents with all follow-up regarding entitlements, appointments, outpatient programs and other activities. Collaborate with mental health service providers and/or outpatient substance abuse treatment programs. Assist consumers with development of program solving and anger management techniques. Maintain, secure and complete appropriate documentation in resident’s charts. Attend and participate in clinical and staff meetings. Conduct home visits and assess resident’s daily living skill and assist when needed. Conduct group activities in skills building such as budgeting, financial planning, patenting, how to complain, maintaining a well-kept home, etc… Assist clients with the development of his/her own support system by connecting him/her with available neighborhood resources, e.g., substance abuse/mental health/therapy, public service personnel, community based organization programs/services, family ties. Establishes goals and objectives with consumers for ADL and may be assigned other tasks or duties reasonably related to the job.
QUALIFICATIONS: Minimum Associates Degree required with at least 1 year experience working with the mental health consumers. Candidate must have basic computer competency and good written and verbal communication as well as the ability to recognize confidentiality of information and to maintain such confidences also necessary.
