Case Manager - Court-based Intervention and Resource Team (CIRT)

Reports to: Clinical Supervisor

Overview: The Center for Alternative Sentencing & Employment Services (CASES) is a private nonprofit serving NYC residents involved in the criminal justice system and those with mental illness who have not engaged in traditional mental health services. The Manhattan Court-based Intervention Resource Team (CIRT) provides alternative to detention (ATD) and alternative to incarceration (ATI) services for youth and adults with mental health problems, serious mental illness, and co-occurring disorders diverted in the Manhattan Criminal and Supreme Court. The case manager provides case management, group-work, and rehabilitation services to promote participant completion of ATD and ATI supervision. The case manager provides services to individuals within the framework of the core alternative sentencing principles of participant self-determination, recovery, cultural competency, and responsivity to criminal behavior. The case manager works with participants with a range of need and risk factors for substance abuse and criminal behavior. Using evidence-based approaches to engagement, public health, substance abuse, mental illness, and criminal behavior supports participants to reduce criminal recidivism and access community resources.



Responsibilities:

· Manage caseload of 25-27 CIRT participants and provide an average of 65-70 face-to-face units of service per month in the program office and also in community with location of service delivery determined by participant need

· Conduct onsite program groups according to training and competence to engage participants in onsite services

· Provide intensive case management services by communicating with community treatment providers, conducting site visits, and attending case conferences to engage and support participants in community treatment

· Assess participants immediate needs, legal circumstances, and engagement needs and using motivational interviewing identify risk factors for criminal recidivism, inform participant of risk factors and identify community resources to support participant to reduce dynamic risk factors and address needs

· Act as advocate and liaison for participants in securing access to community services, income support, and natural supports such as self-help

· Complete charting using database following program standards and timelines

· Involve family, significant others, and treatment providers in service provision

· Provide coverage for late night reporting schedule to support participants engaged in employment, vocational training and education have adequate access to CIRT services

· Provide culturally competent services in accordance with CASES policies and practice

· Attend and prepare for supervision sessions

· Develop and maintain strategic alliances with community providers

· Any other duties as required by the supervisor and directors

Qualifications

· Bachelor’s degree of Social Work or related social service field

· Minimum of two years of experience working in human services with individuals with mental illness, substance use disorders, co-occurring mental health and substance use disorders and involvement in the criminal justice system.

· Experience understanding various models and theories pertaining to treatment of substance abuse, mental illness

· Willingness to learn and adopt evidence-based principles to reduce recidivism risks

· At least one year of experience conducting group sessions using evidence-based practices

· Extensive knowledge of community treatment, support services, and resources

· Highly organized and self-motivated

· Proficient in computers

· Fluency in Spanish preferred
