Community Resource Specialist
The Bridge, an innovative and well respected mental health agency serving adults with serious mental illness, substance abuse, co-occurring mental health and substance abuse disorders, and co-morbid medical conditions is seeking a Community Resource Specialist who will be responsible for providing supportive services to formerly homeless adults diagnosed with serious mental illness and/or substance abuse disorders who are living in independent apartments; developing and supervising activities and groups for clients both on and off site; documenting interactions with clients in AWARDS and according to funder requirements; participating in service planning and assists Case Manager with development and updating psychosocial and functional assessments; assisting and encouraging clients with ADLs, travel, appointments, and shopping; educating clients about services available within the community, assisting in determining eligibility for benefits and services, facilitating enrollment (benefits, Veterans services, education, volunteerism, etc.). Maintaining community involvement to identify community needs and enhance working relationships with community resources, groups and individuals. Other duties as assigned.
Associates Degree or Equivalent with a minimum 2 years' experience working in a supportive residential program and/or with individuals with serious mental illness and backgrounds homelessness. Computer literate with knowledge of Microsoft Office, and spreadsheets. Knowledge of AWARDS preferred.
