Case Manager - Lee

The Center for Urban Community Services, a national leader in the development of effective housing and service initiatives for homeless people, invites applications for a Case Manager position at the Lee. The Lee, located in the Lower East Side, is a 208 unit supportive housing residence for low income tenants, many of whom will have histories of mental illness, homelessness, substance abuse, and/or HIV/AIDS, is seeking a Case Manager. We serve a population of 207 clients who have been formerly street homeless with either active mental illness or substance use. We strive to help all clients live full and satisfying lives in the community.

The Case Manager is responsible for:

Working with an assigned group of roughly 25-30 special needs and low-income clients

Collaborating with colleagues and team mates to meet contractual obligations and program success

Utilizing core clinical principles to assist clients in their move towards fuller and more satisfying lives

Coordinating with outside providers to ensure holistic care for clients

Providing strengths-based advocacy and collaboration with clients

Fulfilling CUCS’ core purpose, core values, and vision statement


Case Manager 1 – HSD + 2 years experience

Case Manager 2 – Bachelor’s Degree or HS Diploma or equivalent and 4 years relevant experience.

Case Manager 3 - BSW and 1 year relevant experience (excluding fieldwork) or Bachelor’s degree and 2 years relevant experience or HS diploma or equivalent and 6 years relevant experience.

For applicants without college degrees, every 30 credits can be substituted for 1 year of experience.

· Strong written and verbal communications skills

· Computer literacy required

· Familiarity with mental health, homelessness, and/or substance use services.

· Must be able to work effectively as a part of a team.

Bilingual English/Spanish preferred
