Per Diem Case Manager (CPE)

Description

The Crotona Park East is a 60 unit studio apartment building for people with serious mental illness returning to the community. The building is newly-constructed and includes many amenities found in luxury buildings including an on-site gym and a fully-equipped computer room. Licensed by the New York State Office of Mental Health as a CR-SRO, the Crotona Park East is extended stay housing where residence is from two to five years. The program’s goal is to work with residents to prepare for and obtain less service-intensive housing in the community. The program provides extensive direct services including Evidence-Based Practices (EBPs). These include Supported Employment, Wellness Self-Management and Behavioral Treatment for Substance Abuse.

Responsibilities: The Per Diem Case Manager’s responsibilities will include case management and clinical services, engagement, outreach, psycho-education, coordination of care, ongoing individual counseling, advocacy, as well as complete documentation of these services. The Per Diem Case Manager will participate and contribute to a program culture that is client-centered, outcome-oriented and dedicated to continuous quality improvement. The Per Diem Case Managers primary tasks are medication monitoring and crisis management.

The Per Diem Case Manager shifts are primarily on the weekends, holidays, evenings and limited weekday hours. Shifts are 7am-4pm and 4pm-12am. Per Diem Case Managers may cover weekday hours if needed. Per Diem Case Managers are expected to cover any unplanned absences, and work minimally four to six scheduled shifts a month and five holidays yearly.

Qualifications

High school diploma and 4 years experience or a Bachelor’s Degree preferred
Good verbal and written communication skills
Computer literacy
Bilingual Spanish/English preferred
Experience with medication monitoring preferred
