Title: Intake Coordinator
Unit: Alternatives to Incarceration
Reports to: Program Supervisor
Status: Full Time; Regular; Non-Exempt

Location: Long Island City
Days/Hours: Monday – Friday 9:00am – 5:00pm

Position Summary:
Responsible for screening, completion of all assessment tools, counseling sessions, monitoring clients attendance and participation, and make referrals internally and externally for all new clients entering or returning to the program.

Essential Duties and Responsibilities:
• Complete full intake assessments for clients that are being admitted into the program;
• Enter and record all intake data into our data base(s);
• Encourage and assist clients in planning, prioritizing and achieving productive long-term and short-term goals;
• Perform general Counseling/Case Management duties, including weekly individual sessions with clients, developing client schedules, and initial risk assessment (LS-CMI);
• Assist clients in identifying and prioritizing needs;
• Generate detailed individual Progress Notes highlighting all pertinent client information;
• Advocate on client’s behalf and schedule internal and external referrals as required;
• Perform crisis intervention as needed;
• Communicate and interact positively with clients and staff;
• Maintain accurate and timely documentation;
• Facilitate groups as needed;
• Ensure accurate and timely data entry of all client information into the agency’s database;
• Provide accurate and timely data and information to be included in court reports;
• Monitor participants attendance and compliance with mandate requirements;
• Submit Program Standards for a client’s non-compliance in a timely fashion;
• Conduct toxicology for clients as needed;
• Attend and contribute to weekly supervisions with the ATI Program Supervisor;
• Participate in client case conferences for optimum collaboration among departments/programs and external agencies;
• Attend and participate in meetings, conferences, and trainings;
• Ensure clients charts are in compliance and ready for transfer within 30 days of intake;
• Provide listing of case transfers to the ATI Program Supervisor in a timely fashion;
• Develop detailed comprehensive discharge plans for clients prior to their discharge dates; and
• Additional responsibilities as required.



Qualifications:

• High school diploma or GED/HSE required; Bachelor’s degree or higher desired;
• Experience and commitment to working with mandated participants along with substance abusers and criminal justice involved individuals;
• Strong Microsoft Office and database entry skills;
• Strong assessment and engagement skills;
• Strong written and verbal communication skills;
• Bilingual English/Spanish a plus;
• Strong organizational and leadership skills; and

Travel Requirements: May require travel to our Harlem office.
