- Develop and implement classes, groups and activities whose focus is rehabilitation assessment, engagement and development in PROS (Personalized Recovery Oriented Services). Classes, groups and activities are designed to facilitate the recovery of individuals diagnosed with mental illness and mental illness conditions.
-Assist/empower individuals to identify and achieve recovery goals of employment, education and community integration.
-Facilitate orientation and complete intake assessments.
-Knowledge using Electronic Health Records.
-Maintain a caseload of individuals for the purpose of engagement, assessment and rehabilitation planning and goal setting acquisition.
-Complete documentation according to OMH guidelines.
-Provide individual and group rehabilitation/recovery and guidance/empowerment in PROS program.
-Provide coverage of programming usually facilitated by other staff that may not be scheduled to work on any individual day.
-Receive supervision by and report to the Program Director/Supervisor.
-Function as a vital part of an interdisciplinary team environment.
-Master' degree in social work or mental health counseling, LMSW, LMHC or LCSW preferred or bachelor's degree with 5 years of behavioral health experience.
-Knowledge of using electronic health records a plus.
